Insurance Information

Prior to your first session, please call the Customer Service number on your insurance card and ask the following questions:

• Does my policy cover out-patient, mental health treatment?
• How much is my deductible and have I met it?
• How much  is my co-pay?
• Is there a limit to the number of sessions that my plan will cover?
• If my counselor is not “in-network” with my insurance plan, does my plan offer out-of-network benefits?
• Is pre-authorization required for out-patient mental health benefits?
